Need your ID card?

We’ve made it very easy for you to view, print, or request a copy of your member identification (ID) cards!

First, be sure to register and log into your account. Then, under “My Benefits” in the left-hand navigation bar, click on “Request a New ID Card."

Below is a sample of what your ID cards might look like:

You can quickly print a temporary card or access a digital version on your smartphone by clicking on the blue tabs under “Print Temporary ID Cards.” 
*Note: You may have to disable your pop-up blockers.

Additional ID cards

You ONLY need to request new ID cards if you:
  • Did not receive your initial TeamCare ID card
  • Lost your card
  • Need additional cards for family members

In the "Request Permanent ID Cards” section just below the blue tabs, choose the cards you need, verify your mailing address, and click the Submit button. Your cards will be mailed to you within 7-10 days.

You DO NOT need to request a new card (because one will automatically be sent) if:
  • You are a new member
  • Your name changes
  • You move to a new network
  • You change plans and your co-pay is different
Remember: Requests for ID cards will not be sent to minors or the parent of a minor if sufficient documentation is not on file with TeamCare.

Also, you will only be able to request ID cards if you have current coverage under a TeamCare benefit plan.
