SSNs requested to meet Federal reporting requirements

You may have received a mailing recently from TeamCare requesting social security numbers (SSNs) for one or more of your dependents covered by TeamCare any time during 2015.

The mailing was only sent to TeamCare participants whose family profile did not already contain this information. If you did not receive a letter, your family profile is already up to date.

Some of you may be cautious to provide social security numbers; please understand we are required by law to have this information for reporting purposes under the Patient Protection and Affordable Care Act (ACA) and Mandatory Insurer Reporting Law (MIR). Also, if your covered spouse is receiving Medicare benefits, TeamCare will need their Medicare Health Insurance Claim Number (HICN) to coordinate coverage with Medicare.

To ensure benefits are not disrupted, please provide the requested information by Sept. 10, 2015. You can return the form in the postage-paid envelope included with the letter; mail it to TeamCare, PO Box 5112, Des Plaines, IL 60017-5112; fax the form to 1-847-518-9784, or call TeamCare at 1-800-TEAMCARE (1-800-832-6227).

Rest assured, this information will only be used for internal processing purposes and shared with the government under the guidelines required by law.
