TeamCare providers have agreed to accept pre-negotiated rates for services they provide to you or your covered dependents. This ensures that you receive the lowest possible costs available in your area.

 

 

 

Wellness

We recognize the importance of regular routine checkups and physical examinations. Your TeamCare Wellness benefit offers you and your family this important coverage. Adult wellness coverage includes an annual physical exam, routine screenings, and a women's health benefit. Children's wellness coverage includes well-baby care, physical examinations, and immunizations — all covered in full.

Lab

TeamCare has joined with Quest Diagnostics to offer the TeamCare Lab Benefit, which allows you and your family to save money on outpatient lab testing.

TeamCare Lab Benefit is a voluntary program that offers outpatient laboratory testing at no cost when testing is performed through Quest Diagnostics. Click here for more information on the Lab Card Program or visit labcard.com.

MRI/CT/PET

TeamCare has joined with US Imaging to provide members with access to over 2,200 facilities that perform MRIs, CTs, and PET scans.

If you and your covered dependents elect to use this voluntary program for your covered outpatient advanced radiology procedures, you pay no deductible or co-insurance towards these services when scheduled through US Imaging.

$0 CVS MinuteClinic Visits

TeamCare Participants are eligible for a $0 office co-pay for covered services when using a CVS MinuteClinic® for common illnesses and minor injuries. Present your TeamCare ID Card when you or your covered dependents visit the clinic inside pharmacy stores. There is no appointment necessary, and they are open seven days a week, including evenings.

CVS MinuteClinic Practitioners can:

  • Treat common illnesses, like strep throat, earache, pink eye and sinus infections.
  • Treat minor injuries and skin conditions.
  • Write prescriptions when appropriate.
  • Treat patients 18 months and older.

Conifer Health Solutions Personal Health Management Program

TeamCare has teamed with Conifer Health Solutions to oversee your continued health care. If you are a candidate for personal health management services, a nurse will call to provide information about the program and discuss your healthcare needs.

Your willingness to participate in the Conifer Health Solutions Personal Health Management Program is completely voluntary and the key to achieving a successful outcome.

Mayo Clinic Services and Ask Mayo Clinic Hotline

TeamCare has joined with Mayo Clinic—a leader in quality medical care for more than a century. You and your family have direct access to Mayo Clinic for any medical care—whether it is for information, a second opinion or the actual treatment.

When you have more questions than answers about your health, you can now pick up the phone and call Ask Mayo Clinic—at any time, day or night. Call 1-800-700-MAYO (6296).

Family Protection Benefit

With most health plans, a family’s benefits will come to an end in the event of the Member's death. Typically, the only way to continue coverage is to make COBRA self-payments which, for a family, can be very costly at a difficult time.

Under the Family Protection Benefit, your covered spouse and family members will be eligible for up to five years of continued health coverage at no cost. If qualified, covered family members will receive the same TeamCare benefits that a Member had prior to their death.

Medical ID Card

Quickly print a temporary ID card, request a new ID card, or use your smartphone to immediately access and use a digital version of your ID card.

From your desktop computer, log in to your account and select "Request a New ID Card" from your dashboard. From your mobile device, log in to your account and select "My ID Card" from the menu.
