Short-Term Disability

When you’re unable to work due to a non-work-related injury, a pregnancy, or recovering from an illness, TeamCare’s Short-Term Disability Benefit is here to take care of you.

    To apply for short-term disability, you must be:
    • Disabled as a result of a non-work-related injury or illness
    • Unable to work due to pregnancy
    • Following a treatment plan from a physician
    • Actively employed and covered by TeamCare when you become disabled

    We know that this time in your life can be overwhelming, which is why we've put together a step-by-step how-to guide for filing for short-term disability benefits. This guide provides everything you need to know about completing and submitting short-term disability forms to TeamCare.

    After your disability payments begin, and depending on the length of time you are out of work, you may be asked to submit a Short-Term Disability - Continuation Form to TeamCare with updates from your physician. You can download and complete the Short-Term Disability - Continuation Form by visiting Forms & Documents. To avoid any delays in processing your payments or extending your coverage, please submit all required forms as soon as possible.
