When Can a Dependent Be Added?
During your initial enrollment into TeamCare: You can add any dependents you may have when you first enroll in TeamCare.During Open Enrollment: The Open Enrollment period takes place annually during the fall, if your plan has different levels of coverage (tiers). Any changes you make during Open Enrollment will take effect at the start of the next calendar year. If you have no changes to your dependents or plan, you don’t have to do anything during Open Enrollment.
Through Special Enrollment: If you have a qualifying event (marriage, having a baby, spouse lost health coverage, etc.) outside of the Open Enrollment period and you would like to add or remove a dependent from your plan, you can do so through Special Enrollment. You must notify TeamCare within 60 days of that event.
What Documents Do I Need to Add a Dependent?
What documents you need to submit depends on who you are adding to your plan.

How Do I Submit My Documents?
Make sure your name and UMI (your unique member ID, a number which likely starts with 806 that can be found on your TeamCare card) are on every document.

Via the Message Center
To send in your documents electronically we recommend scanning them and sending via the Message Center.
To mail in your documents, please include the cover sheet from your enrollment packet and send them to:

Please note: If you choose to take a photo of the document with your phone and send it in via the Message Center, please make sure the document is legible. Double check the image to be sure nothing is cut off or blurry.
