How to Add a Dependent

Getting your dependents, such as children or a spouse, covered under your TeamCare plan may require submission of certain legal documents. Use this guide to find out what documents we may need from you during the enrollment process.
Remember, claims cannot be paid until we receive all the required documents!

When Can a Dependent Be Added?

During your initial enrollment into TeamCare: You can add any dependents you may have when you first enroll in TeamCare.

During Open Enrollment: The Open Enrollment period takes place annually during the fall, if your plan has different levels of coverage (tiers). Any changes you make during Open Enrollment will take effect at the start of the next calendar year. If you have no changes to your dependents or plan, you don’t have to do anything during Open Enrollment.

Through Special Enrollment: If you have a qualifying event (marriage, having a baby, spouse lost health coverage, etc.) outside of the Open Enrollment period and you would like to add or remove a dependent from your plan, you can do so through Special Enrollment. You must notify TeamCare within 60 days of that event.
What Documents Do I Need to Add a Dependent?

What documents you need to submit depends on who you are adding to your plan.

If you're enrolling: You'll need:
Just me
  • All we need from you is to complete the employee information section on your information form, or through the enrollment process online.
My spouse and I
  • You’ll need to complete the employee and spouse information sections on the enrollment form or online process, including the social security numbers for both you and your spouse. This information is required for Coordination of Benefits purposes even if spouse coverage is not elected. In certain cases, your marriage certificate may be requested as well.
  • If you get married outside of the Open Enrollment period, you must notify us of your marriage within 60 days in order for your spouse to receive coverage.
My spouse, my children and I
  • For your family to receive coverage you’ll need to complete information sections for yourself, your spouse, and each child on the enrollment form or online process, including the social security numbers for all individuals. For your spouse, a marriage certificate may be requested as well.

    If your child is born after your initial enrollment but outside of the annual Open Enrollment period, you must notify TeamCare within 60 days of the child’s birth to add them as a dependent.

If you have: You'll need:
A dependent child from a previous marriage
  • The complete Divorce Decree & Settlement of the natural parents.
A stepchild/stepchildren
  • The child or children’s birth certificate, the complete Divorce Decree & Settlement of the natural parents, and a marriage certificate to your current spouse.
A child born outside of marriage
  • A court order regarding insurance, the child’s birth certificate, and the name and birth date of other natural parent, including information regarding any insurance coverage.

  • If your child is born after your initial enrollment but outside of the annual Open Enrollment period, you must notify TeamCare within 60 days of the child’s birth to add them as a dependent.
A child for which you are guardian
  • Guardianship/custody documents.
Adopted child
  • Final adoption papers. If the adoption is not yet final, please provide a copy of the Placement Agreement.

  • If you adopt a child after your initial enrollment but outside of the annual Open Enrollment period, you must notify TeamCare within 60 days of the adoption to add them as a dependent.
Adult child
  • Birth certificate of the child.

How Do I Submit My Documents?

Make sure your name and UMI (your unique member ID, a number which likely starts with 806 that can be found on your TeamCare card) are on every document.

Please note: If you choose to take a photo of the document with your phone and send it in via the Message Center, please make sure the document is legible. Double check the image to be sure nothing is cut off or blurry.
